Seeking Doctor or Medical Treatment with Your Insurance Plan

If you need to see a doctor, always call Hollard Travel/United Healthcare at 1-877-826-2910 BEFORE and AFTER seeking medical attention.

 

 

When you call Hollard Travel, please give them the following information:

• First and last name

• The nature of your illness or the reason you are seeking medical attention

 

Hollard Travel will provide you with information for a nearby doctor’s office that accepts the insurance.
Make sure to write down the following information:
• Address and a phone number of the doctor’s office
 • Name of the person that you spoke with at Hollard Travel/On Call International

 

When you go to see a doctor, be sure to bring:

• Your passport 

• Your insurance information 

• Money (checkbook, debit/credit card) to cover the co-pay

Request a copy of all forms you sign or that are given to you regarding your medical visit, including your medical records.

 

In United States of America: Urgent care centres are different from emergency rooms.

You MUST visit an URGENT CARE CENTRE before you go to the emergency room, unless you are unconscious, or it is a life-threatening medical condition that requires inpatient treatment.

 

 You MUST call for authorization BEFORE you visit an Emergency Room. Hollard Travel do NOT cover any specialist tests or any treatment in the ER if not authorized by Hollard Travel.

 

Hollard Travel does NOT cover follow-up treatment, or chronic medication.

Hollard Travel does NOT cover any claims related to pregnancy.

 

Health: Your policy contains restrictions regarding Pre-existing Medical Conditions. You must tell Hollard Travel if you have any chronic medical conditions or any conditions that were treated before this policy was issued. If you don’t tell Hollard Travel, then the condition is not covered.

 

Policy Excess: Claims are subject to an excess. This means that you MUST pay the excess amount to the hospital or doctor BEFORE you receive treatment.

Calling BEFORE receiving medical attention ensures you find out:
• If the treatment you need is covered by your insurance.
• If the clinic or doctor you want to visit is in-network.
  • If your doctor’s visit requires pre-approval.

 

Calling within 10 days AFTER receiving medical attention ensures:
• The insurance claim has been properly filed.
• You find out exactly what you need to submit to be reimbursed for medical expenses.
• You open a “case” in the instance that your medical issue requires multiple doctor’s
  office visits.

 

Medical Emergency: Accident & emergency department (A&E), emergency room (ER) or casualty department.
• You, your travel companion, or someone you know must phone 1-877-826-2910 BEFORE you consult with a medical doctor, BEFORE any tests and BEFORE you are admitted as an inpatient.

 • If you are admitted as an inpatient, or any tests are done and you don’t call, the maximum insurance will pay is $1,500

 

All Claims must be received by Hollard Travel within 30 days.
 Please note if you fail to contact Hollard Travel before receiving medical attention (unless an emergency), you may be responsible for all medical bills.

 

Deductible or Co-pay
This means that you MUST pay the excess amount to the hospital or doctor BEFORE you receive treatment.
If you seek medical attention while on your program, you will have to pay a deductible (co-pay). Deductible/co-pay is a fee that is normal for all insurance companies. You will not get back the money you paid for the co-pay.
• Co-pay for doctor’s office visits and clinics- $100
• Emergency Room/Hospital visit- $250. If you are hospitalized, you will not have to pay a $250 deductible.
 Please make sure to go to the Hospital ER only in case of a true life-threatening emergency.
